Introducción y objetivos: La hiponatremia es un factor de mal pronóstico en pacientes con insuficiencia cardiaca. Nuestro objetivo fue valorar el impacto de la hiponatremia en la evolución de los pacientes sometidos a implante de desfibrilador en prevención primaria. Método: Se trata de un estudio observacional retrospectivo multicéntrico en el que se valoró la natremia periimplante y los eventos adversos (la mortalidad de cualquier causa y la cardiovascular, el primer ingreso por insuficiencia cardiaca, la primera terapia apropiada e inapropiada del desfibrilador), en una cohorte española sometida a implante de desfibrilador en la prevención primaria. Se comparó la hiponatremia ligera (Na ≤ 135 mEq/l) y la moderada-severa (Na ≤ 131 mEq/l) respecto a la ausencia de la hiponatremia. Resultados: Un total de 725 pacientes, con un valor medio de sodio en la sangre periimplante de 139 ± 3,6 mEq/l y un tiempo medio de seguimiento de 34,1 ± 18,5 meses. En los pacientes con Na ≤ 135 mEq/l (n = 105) se observó una mayor incidencia de primer ingreso por insuficiencia cardiaca (odds ratio: 1,701; p = 0,031). En los pacientes con hiponatremia moderada-severa (n = 18) se observó mayor mortalidad total (hazard ratio; 3,552; p = 0,002) y mayor mortalidad cardiovascular (hazard ratio: 4,24; p = 0,003). Ninguno de los grados de la hiponatremia estudiados se asoció con mayor incidencia de terapias apropiadas o inapropiadas. Conclusiones: La hiponatremia ligera en el momento del implante de un desfibrilador se asocia a una mayor incidencia de ingreso por insuficiencia cardiaca en el seguimiento, mientras que en el caso de una hiponatremia moderada-severa, se asocia a un riesgo de muerte 4 veces mayor.


Introduction and objectives: Hyponatremia is a prognostic factor in patients with heart failure. Our objective was to assess the impact of hyponatremia in the evolution of patients undergoing cardioverter implantation in primary prevention. Method: This is a multicenter retrospective observational study in which serum sodium periimplant and adverse events (all-cause and cardiovascular mortality, first admission for heart failure, first appropriate and inappropriate therapy) were assessed in a Spanish cohort receiving cardioverter in primary prevention. Patients with mild hyponatremia (Na ≤ 135 mEq/L) and those with moderate to severe hyponatremia (Na ≤ 131 mEq/L) were compared to those without hyponatremia. Results: 725 patients, with a mean value of sodium in blood peri-implant of 139 ± 3.6 mEq/L and a mean follow-up was 34.1 ± 18.5 months. In patients with Na ≤ 135 mEq/L (n = 105), an increased incidence of first admission for heart failure was observed (Odds Ratio: 1.701; P = .031). In patients with moderate to severe hyponatremia (n = 18), higher total mortality (Hazard Ratio: 3.552; P = .002) and increased cardiovascular mortality (Hazard Ratio: 4.24; P = .003) were observed. None of those values of hyponatremia were associated with increased incidence of appropriate or inappropriate therapies Conclusions: Mild hyponatremia at the time of cardioverter implantation is related with a higher incidence of hospitalization for heart failure during follow-up, while moderate to severe hyponatremia increases the risk of death during follow-up.

AIMS: The benefit of implantable cardioverter-defibrillators (ICDs) in patients at risk of sudden death has been established in randomized clinical trials (RCTs) using the ICD models available at the time. However, observational large-scale data on the incidence of arrhythmias in up-to-date ICDs implanted according to the current guidelines are scarce. The aim was to assess the incidence of arrhythmias in a large, current ICD population based on a blinded peer review of the detected episodes. METHODS AND RESULTS: UMBRELLA is a multicentre, observational registry of ICD patients followed by remote monitoring. Stored episodes were classified by a blinded committee of experts. Subgroup analyses were based on clinical profiles established by previous pivotal RCTs of ICDs. Of 1514 enrolled patients, 605 (39.9%) patients had 5951 episodes after 26 ± 17 months follow-up, being 3353 of them (56.3%) sustained ventricular arrhythmias (SVA), and 13.2% of SVA were self-terminated. Appropriate and inappropriate shocks occurred in 11.6 and 5% of patients, respectively. The 3 years cumulative incidence of SVA was 25% (95% CI: 21-28%) in primary prevention patients and 41% (95% CI: 36-47%) in secondary prevention patients (P < 0.001). Male gender, secondary prevention, and atrial fibrillation as basal rhythm were significantly related to a higher incidence of SVA. CONCLUSION: This real-world analysis suggests that modern ICD patients have a low rate of appropriate and inappropriate shocks. The risk of SVA in secondary prevention patients is less than what has been reported in RCTs.

Currently, nuclear magnetic resonance imaging is contraindicated in patients with a pacemaker or implantable cardioverter-defibrillator. This study was carried out because the potential risks in this situation need to be clearly defined. This prospective study evaluated clinical and electrical parameters before and after magnetic resonance imaging was performed in 33 patients (five with implantable cardioverter-defibrillators and 28 with pacemakers). In these patients, magnetic resonance imaging was considered clinically essential. There were no clinical complications. There was a temporary communication failure in two cases, sensing errors during imaging in two cases, and a safety signal was generated in one pacemaker at the maximum magnetic resonance frequency and output level. There were no technical restrictions on imaging nor were there any permanent changes in the performance of the cardiac pacing device.

RESUMO

La resonancia magnética está actualmente contraindicada a los pacientes portadores de dispositivos de estimulación cardiaca. Ante la necesidad de concretar los riesgos potenciales derivados de esta situación, surgió este estudio. De forma prospectiva, se evaluaron parámetros clínicos, eléctricos y técnicos antes y después de la realización de la prueba en 33 pacientes (5 desfibriladores automáticos implantables, 28 marcapasos), en los que se consideró clínicamente indispensable realizarla. No se apreciaron complicaciones clínicas. Se detectaron dos casos de fallo temporal de telemetría, dos errores de detección durante la exploración y una respuesta de seguridad en un marcapasos a frecuencia magnética y salida máxima. No hubo limitaciones técnicas en la adquisición de imágenes ni alteraciones permanentes del funcionamiento de los dispositivos de estimulación cardiaca (AU)

INTRODUCTION AND OBJECTIVES: The aim of this article was to report findings from the Spanish Pacemaker Registry on procedures carried out in 2007. The analysis includes details of patients' demographic characteristics, the type of procedure (i.e., implant or replacement), pacing mode, and the electrode and fixation systems used. METHODS: The data collected comprised all the information recorded on European Pacemaker Patient Identification Cards, which were submitted voluntarily in either electronic or paper form. RESULTS: The information received from 114 health-care centers included 11 360 cards, which covered 36.9% of all pacemakers thought to be implanted during 2007. Some 680.4 pacemakers were used per million inhabitants. Usage varied significantly between different Spanish autonomous regions, partly due to uneven ageing of the population. Some 25.3% of all procedures were to replace generators. The average age of patients undergoing a first implantation was 76.1 years, with a difference between the sexes: 75.4 years in men and 77.1 years in women. First implantations were carried out most often in patients in their 70s, who comprised 40% of the total, compared with other decades of life. There were more first implantations in men, who accounted for 57.9% of the total, due to a higher incidence of intraventricular conduction disturbance. In contrast, sick sinus syndrome was equally common in both sexes. Leaving aside patients with atrial tachyarrhythmia, it was observed that the VVI/R mode was used in 26.2% of those with sick sinus syndrome, 28% with atrioventricular block and 28.1% with intraventricular conduction disturbance. Age had a great influence on whether this pacing mode was selected. Almost all leads used were bipolar. The percentage using active fixation increased to 36.5%. The percentage of implants involving low-energy cardiac resynchronization therapy remained unchanged. CONCLUSIONS: The Spanish Pacemaker Registry received data on a highly representative sample of patients requiring cardiac pacing in 2007, which covered 11 360 device implantations or replacements. It was observed that the incidence of first implantations was influenced by sex, being greater in males, who underwent implantation at a younger age because conduction disturbances were more common. Age was one factor influencing the pacing mode selected for a range of electrocardiographic indications for pacing. Almost all leads implanted were bipolar. The percentage using active fixation continues to increase.

Introducción y objetivos. En este artículo se presentan los resultados del Registro Español de Marcapasos correspondientes a la actividad realizada en el año 2007, analizándose tanto el tipo de proceso (implantes y recambios) como su distribución poblacional, modos de estimulación y los sistemas de cables-electrodos utilizados. Métodos. La recogida de datos se basa en la información que aporta la Tarjeta Europea del Paciente Portador de Marcapasos, la cual es remitida de forma voluntaria en formato magnético o mediante copia de la tarjeta. Resultados. Se recibió información de 114 centros hospitalarios, con un total de 11.360 tarjetas, que suponen el 36,9% de todos los marcapasos que se estima fueron utilizados. El número de unidades consumidas por millón de habitantes fue 680,4; se aprecia una significativa variación entre las diversas comunidades autónomas, en parte debido al desigual envejecimiento poblacional en ellas. El 25,3% de la actividad referida al registro corresponde a recambios de generadores. La media de edad de los pacientes que reciben su primer implante fue 76,1 años, con diferencias en función del sexo: 75,4 años en varones y 77,1 años en mujeres. La década en la que se objetiva un mayor número de primoimplantes corresponde a los 70 años, con el 40% del total. La frecuencia de primoimplantes fue significativamente superior en los varones (57,9%), por una mayor incidencia en éstos de los trastornos de la conducción; sin embargo, es similar en ambos sexos la enfermedad del nódulo sinusal. Excluidos los pacientes en taquiarritmia auricular, se aprecia que el 26,2% de los pacientes con enfermedad del nódulo sinusal, el 28% de los bloqueos auriculoventriculares y el 28,1% de los trastornos de la conducción intraventricular son estimulados en modo VVI/R. La edad es un factor muy influyente en la selección de este modo. La práctica totalidad de los cables endocárdicos utilizados son bipolares. Se percibe un aumento del porcentaje de los que utilizan un sistema de fijación activo, que supone el 36,5%. La estimulación para la terapia de resincronización ventricular de baja energía se mantiene estable en número total de implantes. Conclusiones. El Registro Español de Marcapasos recoge una muestra prospectiva muy representativa de la estimulación realizada en el año 2007, con un total del 11.360 implantes y recambios. Se objetiva que el sexo del paciente influye en la incidencia de los primoimplantes, que es mayor en los varones y a una edad más temprana debido a una mayor frecuencia de las alteraciones de la conducción en ellos. La edad es un factor influyente en la selección del modo de estimulación en las diversas indicaciones electrocardiográficas que originan el implante. Casi todos los cables utilizados son bipolares, y aumenta evolutivamente el porcentaje de los que utilizan un sistema de fijación activo (AU)

RESUMO

This review summarizes recent scientific developments in ambulatory monitoring and its legal implications in appropriately selected patients. It also considers how new cardiac pacing devices can be used in the clinical monitoring of atrial fibrillation, sleep apnea and, in transplant patients, heart failure and transplant rejection. In addition, the system of accreditation for cardiac pacing services in Spain is outlined, with a focus on process development and quality indicators. Finally, the scientific articles on cardiac pacing that made the greatest impact in 2007 are summarized.

This article summarizes progress in cardiac pacing that has taken place in the last year, such as the incorporation of navigation technology into programmers, systems for monitoring hemodynamic parameters, and new developments in patient follow-up, principally home monitoring and the use of treatment guidelines. In addition, the article covers recent findings in resynchronization therapy, paroxysmal atrial fibrillation, and the management of patients with pacemakers who need to undergo magnetic resonance imaging.

Asimismo, se abordan los avances en resincronización, fibrilación auricular paroxística o el tratamiento actual de los pacientes portadores de marcapasos a los que se les desea realizar una resonancia magnética

INTRODUCTION AND OBJECTIVES: Immune response-mediated regulation of myocardial collagen remains poorly understood. Our objective was to investigate the relationship between ventricular remodeling and immunologic activation in patients with heart failure (HF) by comparing dilated and ischemic cardiomyopathy. METHODS: We studied 94 patients with HF and dilated cardiomyopathy (n=46) or ischemic cardiomyopathy (n=48). We recorded left ventricular (LV) volumes, E/A ratio, and ejection fraction. Plasma concentrations of tumor necrosis factor alpha (TNFalpha), soluble TNFa receptor I (sTNF-RI), sTNF-RII, interleukin-6 (IL-6) and IL-10 were measured. The serum procollagen type-III amino-terminal propeptide (PIIINP) level was also obtained. RESULTS: Ventricular volumes were greater in the dilated cardiomyopathy than in the ischemic cardiomyopathy group (P< .05). However, sTNF-RI, sTNF-RII and PIIINP levels were higher in the ischemic group (P< .05). In this group, there were significant correlations between ventricular volumes and IL-10 and sTNF-RII levels. There was also a significant correlation between PIIINP and sTNF-RII levels (r=0.30; P< .05). In the dilated cardiomyopathy group, there was a significant correlation between ventricular volumes and IL-10 level, and between PIIINP level and IL-6 (r=0.32; P< .05) and sTNF-RII levels (r=0.32; P< .05). Multiple linear regression analysis, which included cytokine levels, age, sex and ventricular function, showed that the sTNF-RII level was an independent predictor of the PIIINP level (adjusted r(2)=0.16; P< .0001) and of ventricular volumes (LV end-systolic volume index, adjusted r(2)=0.034; P< .05; and LV end-diastolic volume index, adjusted r(2)=.048; P< .05) in both groups. CONCLUSIONS: In HF, there is an interaction between proinflammatory cytokines and the extracellular matrix. Immunologic implications vary according to disease etiology. The elevation in proinflammatory cytokine and PIIINP levels is greater in patients with ischemic cardiomyopathy. Multiple regression analysis showed that the sTNF-RII level was an independent predictor of ventricular remodeling.

Introducción y objetivos. No se comprende bien la regulación del colágeno miocárdico mediada por la respuesta inmunitaria. Nuestro objetivo fue determinar las relaciones entre remodelado ventricular y activación inmunitaria en pacientes con insuficiencia cardiaca comparando miocardiopatía isquémica y dilatada. Métodos. Estudiamos a 94 pacientes con insuficiencia cardiaca: miocardiopatía dilatada (n = 46) e isquémica (n = 48). Determinamos volúmenes ventriculares, E/A y FE. Medimos las concentraciones de TNFα, sTNF-RI, sTNF-RII, IL-6 e IL-10 y calculamos los valores de PIIINP. Resultados. Los volúmenes ventriculares en la miocardiopatía dilatada fueron superiores a los del grupo isquémico (p < 0,05). Sin embargo, los valores de sTNF-RI, sTNF-RII y PIIINP fueron más elevados en el grupo isquémico (p < 0,05). En éste, los volúmenes ventriculares se correlacionaron significativamente con IL-10 y sTNF-RII. El PIIINP se correlacionó significativamente con sTNF-RII (r = 0,30; p < 0,05). En el grupo de miocardiopatía dilatada, los volúmenes ventriculares se correlacionaron significativamente con IL-10 y el PIIINP se correlacionó con IL-6 (r = 0,32; p < 0,05) y sTNF-RII (r = 0,32; p < 0,05). La regresión lineal múltiple, que incluyó citocinas, edad, sexo y función ventricular, demuestra que el sTNF-RII es un factor pronóstico independiente del PIIINP (r² ajustada = 0,16; p < 0,0001) y de los volúmenes ventriculares (IVTSVI, r² ajustada = 0,034; p < 0,05; IVTDVI, r² ajustada = 0,048; p < 0,05) en ambos grupos. Conclusiones. En la insuficiencia cardiaca hay una interacción de citocinas proinflamatorias con la matriz extracelular. La implicación inmunitaria es diferente dependiendo de la etiología. Las citocinas proinflamatorias y los valores de PIIINP son más elevados en los pacientes con miocardiopatía isquémica. La regresión múltiple demostró que el sTNF-RII es un factor pronóstico independiente de remodelado ventricular (AU)

Ventricular volumes were greater in the dilated cardiomyopathy than in the ischemic cardiomyopathy group (P<.05). However, sTNF-RI, sTNF-RII and PIIINP levels were higher in the ischemic group (P<.05). In this group, there were significant correlations between ventricular volumes and IL-10 and sTNF-RII levels. There was also a significant correlation between PIIINP and sTNF-RII levels (r=0.30; P<.05). In the dilated cardiomyopathy group, there was a significant correlation between ventricular volumes and IL-10 level, and between PIIINP level and IL-6 (r=0.32; P<.05) and sTNF-RII levels (r=0.32; P<.05). Multiple linear regression analysis, which included cytokine levels, age, sex and ventricular function, showed that the sTNF-RII level was an independent predictor of the PIIINP level (adjusted r²=0.16; P<.0001) and of ventricular volumes (LV end-systolic volume index, adjusted r²=0.034; P<.05; and LV end-diastolic volume index, adjusted r²=.048; P<.05) in both groups. Conclusions. In HF, there is an interaction between proinflammatory cytokines and the extracellular matrix. Immunologic implications vary according to disease etiology. The elevation in proinflammatory cytokine and PIIINP levels is greater in patients with ischemic cardiomyopathy. Multiple regression analysis showed that the sTNF-RII level was an independent predictor of ventricular remodeling (AU)

INTRODUCTION AND OBJECTIVES: Elevated plasma concentrations of big endothelin-1 (big ET-1) are related with severity and prognosis in patients with heart failure, and N-terminal pro-brain natriuretic peptide (NT-proBNP) is a marker of ventricular remodeling. The purpose of this study was to investigate the relationship between plasma levels of big ET-1 and NT-proBNP, and between the former and ventricular function. PATIENTS AND METHOD: We studied 103 patients with heart failure (75 men, mean age 63 [13] years). Each participant completed a questionnaire and underwent Doppler echocardiographic study to measure ejection fraction (EF), mitral flow propagation velocity (Vp) and atrioventricular plane displacement (AVPD). Blood samples were also taken to determine plasma levels of big ET-1, aldosterone and NT-proBNP. RESULTS: For the whole population big ET-1 concentration was 1.03 [0.75] fmol/m, NT-proBNP 619 (307-1328) pg/mL, aldosterone 168 [102] pg/mL, EF 37 [10], Vp 37 [11] cm/s, and AVPD 8.0 [1.7] mm. Plasma big ET-1 correlated positively with plasma NT-proBNP (r=0.50, P<.0001). However, a negative correlation was found between big ET-1 and EF (r=-0.30, P<.01), Vp, (r=-0.30, P<.01) and AVPD (r=-0.21, P<.05). When ET-1 levels were divided into quartiles and the corresponding NT-proBNP, EF, Vp and AVPD values were compared, we found significant differences (ANOVA P<.0001, P<.01, P<.05 and P<.05, respectively). CONCLUSIONS: Plasma levels of big ET-1 correlated positively with NT-proBNP levels and inversely with EF, Vp and AVPD. These findings help clarify the relationships between ventricular function and the neurohormonal activation involved in heart failure, and may aid the search for therapeutic interventions.

Introducción y objetivos. Las concentraciones elevadas de big endotelina 1 (big ET-1) se relacionan con la gravedad y el pronóstico de los pacientes con insuficiencia cardíaca (IC), mientras que N-terminal proBNP (NTproBNP) es un marcador de remodelado ventricular. El objetivo de nuestro estudio fue relacionar los valores debig ET-1 con NT-proBNP y con la función miocárdica. Pacientes y método. Estudiamos a 103 pacientes (75varones; 63 ± 13 años) diagnosticados de IC. Cada participante completó un cuestionario y fue sometido a un estudio eco-Doppler para la medición de la fracción de eyección (FE), la velocidad de propagación del flujo mitral (Vp) y el desplazamiento del plano auriculo ventricular(DPAV). Se realizó una extracción de sangre para determinarlos valores de big ET-1, aldosterona y NT-proBNP. Resultados. Para toda la población, el valor de big ET-1 fue de 1,03 ± 0,75 fmol/ml, el de NT-proBNP de 619(307-1.328) pg/ml, el de aldosterona de 168 ± 102 pg/ml,la FE de 37 ± 10, la Vp de 37 ± 11 cm/s y el DPAV de 8,0± 1,7 mm. La big ET-1 se correlacionó positivamente con el NT-proBNP (r = 0,50; p < 0,0001) e inversamente conla FE (r = -0,30; p < 0,01), la Vp (r = -0,30; p < 0,01) y elDPAV (r = -0,21; p < 0,05). Al dividir la big ET-1 en cuartiles y comparar los valores de NT-proBNP, FE, Vp y DPAV obtuvimos diferencias estadísticamente significativas(ANOVA p < 0,0001, p < 0,01, p < 0,05 y p < 0,05,respectivamente). Conclusiones. Los valores plasmáticos de big ET-1muestran una correlación positiva con los de NT-pro BNP y una correlación inversa con la FE, la Vp y el DPAV. Estos hallazgos ayudan a esclarecer las relaciones entre la función ventricular y la activación neurohormonal en la insuficiencia cardíaca y facilitan la búsqueda de nuevas intervención es terapéuticas
